How to Request a Certified Birth Certificate
In Person
- Complete the Vital Records Request Form*. To save time, you may print out and complete it prior to coming in.
- Valid photo ID (i.e., driver’s license, state ID, or passport) must be provided. We will take a photocopy.
NOTE: If you are unable to produce a valid photo ID, you should call first. There are alternate documents that may be accepted by the clerk. One photo ID will be required in almost all cases. - We accept payments by cash, debit/credit card, check, or money order.
By Mail
- Include a completed Vital Records Request Form* or a letter may be written. The letter would need to contain the parents’ full names, including mother’s maiden name, and the child’s name, birthdate, birthplace and the signature of the person requesting the record. It is also helpful if you include the address that you would like the certificate sent to in the letter.
